I need info on a stock gone bad

I had a 4 shares in Take Out Music.com and it went belly up years ago and I never wrote it off on my taxes and I need to do so, what I need to find out is what I need to do that and if anyone has any info on that stock and when it went out? Thank You

When did you sell the shares exactly?

If the company went under say say 5 years ago then I believe you would have needed to write off the losses that year. I dont think you can write off losses from x years ago unless it was a carryover. Not positive though.
